How Coordinated Care Helps

The Coordinated Care Model helps ease a family’s worries, and a senior’s transition into residential care, in three ways:

  • We bring the personalized health care frail elderly patients need directly to the residential facility where they live. Patients and families experience both convenience and the assurance that their loved one has access to a full array of medical services – from health care evaluations, to diagnostics, to treatment tailored to the individual.
  • Providing care onsite enables us to leverage the collective training, skills and knowledge of everyone involved with a resident’s care – from the Consultative Health and Medicine physician and nurse practitioner team to the onsite caregivers and staff at each facility and the family members who know their loved one best. Coordinated Care also helps prevent the disconnects that sometimes can occur.
  • We contract with certain health plan options that are closely aligned with our mission and The Coordinated Care Model. These health plans encourage us to spend the time necessary to regularly monitor a patient’s medical condition and coordinate with facility staff and communicate with family members.
