Consultative Health and Medicine was founded in 2011 by Dr. Chris Johnson, who has focused his practice on serving the frail elderly since 2000, and five nurse practitioners with extensive training and experience in geriatrics and gerontology. They believed seniors would benefit from having access to a medical practice that concentrated on providing high-quality convenient medical care in a setting that best meets their needs. Today, our eight nurse practitioners and two physicians serve more than 700 seniors who reside in 35 senior communities.
